5-Steps to a Smooth Screening Experience
Use this guide as your checklist to prepare for your interview call with Dr. Aimee. To progress through our program, each step must be successfully completed.

Step 1: Review the Criteria
You must meet our minimum requirements to participate as a gestational carrier or egg donor. Do you meet these criteria?
Gestational Carriers
- Age. You’re between the ages of 21 – 40 (exceptions may be considered if you’re carrying for a friend or family member).
- BMI. Your BMI is under 32. Click here to calculate your BMI.
- Pregnancy History. You’ve had fewer than 5 deliveries and no serious complications, such as toxemia, pre-eclampsia, preterm labor, incompetent cervix, and pregnancy-induced hypertension. All your pregnancies have been healthy.
- Postpartum Depression. You have no history of postpartum depression. Your mental health is extremely important to us.
- Full Partner Support. If you have a partner, they fully support you. Both of you must be screened and sign the consent forms.
- Education. You have a high school diploma or GED equivalent.
- Non-Smoking. You do not smoke or vape, and neither does your partner.
- US Resident. You live in the United States.
- Responsiveness. You commit to being responsive and collaborative throughout this entire process.
Egg Donors
- Age. You’re under 32 years old.
- BMI. Your BMI is under 32. Click here to calculate your BMI.
- AMH. You have an AMH over 2.0.
- Fertility History. You have no known fertility issues such as endometriosis, blocked fallopian tubes, low egg quality, fibroids, and autoimmune diseases.
- Mental Health. You have no current mental illness or psychiatric conditions.
- Education. You have a high school diploma or GED equivalent.
- Non-Smoking. You do not smoke or vape, and neither does your partner.
- US Resident. You live in the United States.
- Responsiveness. You commit to being responsive and collaborative throughout this entire process.
If you answered “YES” to all of these requirements, please continue to Step 2!
Step 2: Create a Patient Portal Account
Our New Patient Coordinator will guide you in setting up your patient account and be your go-to person throughout this screening process.
You’ll receive an email invitation to join our patient portal, nAble. If you can’t find this email, please check your spam folder or email your New Patient Coordinator. If you have a partner, they’ll also receive an email with a personalized link to set up their patient account.

Once logged in, you can set up your account:
Personal Information

Go to the Menu on the left and look for “My Personal Info”
- Update your personal information
- Upload the following items:
- Profile photo
- Valid driver’s license or passport for identification (ensure it hasn’t expired)
Insurance Information

Go to the Menu on the left and look for “My Insurance”
Enter your insurance details and upload your insurance card (front and back).
Medical History

Go to the Menu on the left -> click on “Medical Records” and then “History”
Fill out the medical history questionnaire. It takes about 15 minutes, so please set aside uninterrupted time to complete it.
The questionnaire is detailed for a reason — this information is key for Dr. Aimee to gain a thorough understanding of your medical history.
Step 3: Share Your Surrogacy + Pregnancy History
SKIP this step if you’re an egg donor.
Share your complete surrogacy and pregnancy history by filling out the form below. It’s crucial to include your full history and be as thorough as possible, as missing details will delay the process.
Since the form cannot be saved and returned to later, set aside 15 uninterrupted minutes to complete it thoroughly.
Step 4: Sign Consent Forms (if applicable)
If we need to collect your medical records from previous facilities, we’ll ask you to sign the following consent forms. Here’s an overview of each:

HIPAA Notice of Privacy Practices: This notice ensures patients are informed about their privacy rights and the confidentiality of their health information (signed by patient and partner).

Telemedicine Consent: It ensures patients understand and agree to the conditions under which telemedicine services are provided (signed by patient and partner).

Communication Authorization: Allows patients to receive health information via email and text (signed by patient and partner).

Medical Record Release: We require a comprehensive view of your medical history. These forms ensure the transfer of crucial health information between prior health providers and our clinic (signed by patient and partner).

Arbitration Agreement: Ensures a direct, legally recognized method for settling such disputes under California law (signed by patient and partner).
Step 5: Attend Your Screening Call
After signing your consent forms, your New Patient Coordinator will schedule a 15-minute call with Dr. Aimee. Dr. Aimee’s availability is dynamic, so please confirm your appointment time as soon as possible.
Be ready 10 minutes before your appointment. Dr. Aimee will call you within a 30-minute window at the number you registered in your portal account.
After Your Screening Call
Once you get clearance from Dr. Aimee, we’ll move on to the medical screening process. A Nurse will guide you through your medical evaluation and ensure that we receive legal clearance. Here’s what you can expect:
- Lab Work. Blood work is mandatory for a complete medical review.
- Ultrasound. Dr. Aimee may require an ultrasound.
- Fertility Procedures. Dr. Aimee may require procedures such as an HSG, mock cycle, SIS, etc. (gestational carriers only)
- Psychological Assessment. You’ll undergo a thorough evaluation by a licensed mental health professional to ensure you are well-prepared for the mental and emotional aspects of third-party reproduction.
- Legal Clearance. This is the final step of the process. A legally binding contract will be prepared, outlining the agreement between you and the intended parents.
